"Airlines' requirements if you have assistance or medical needs

Airline forms
If you need any assistance from airline staff or if you have medical needs, the airline may ask you to complete an Incapacitated Passengers Handling Advice (INCAD) form and/or a Medical Information Form (MEDIF). These are standard forms used by many airlines to help staff organise any assistance or equipment you may need during your journey and to decide whether you are fit to fly. With some airlines, the INCAD and MEDIF are two parts of the same form."

"There is an IATA standard medical information form (MEDIF) available from travel agents and airlines for this purpose. The Form is in two parts - Part 1 to be completed by the passenger and/or the agent, and Part 2, when appropriate, to be completed by the passenger's medical adviser. The airline will decide whether it can accommodate the needs and thus accept the passenger, if necessary after further medical examination and certification of fitness to fly."

Medical Information Form (MEDIF)

The form includes wording ; 

"The PHYSICIAN ATTENDING the incapacitated passenger is requested to ANSWER ALL QUESTIONS."

"For attending physician:
Please fill out the fourth page "Medical Information Sheet" of this form. Please determine the fitness of the passenger (patient) for the proposed air travel by taking the whole itinerary and the potential affect on the passenger's (patient's) state of health into consideration. We would appreciate any comment about the passenger's (patient's) condition and suggestion for the proposed travel in the lower remarks space."
